On Friday, Korea University Guro Hospital announced that a team of surgeons successfully performed a single-port robotic surgery for a choledochal cyst on a 22-month-old toddler. The procedure presented unique challenges due to the patient’s smaller organs and blood vessels, as well as limited surgical space, requiring exceptional precision.

Choledochal cysts are rare congenital conditions where the bile duct abnormally expands, forming a sac-like structure. If left untreated, these cysts can lead to serious complications.

Without proper treatment, patients may develop gallstones, cholangitis, or pancreatitis. There’s also an increased risk of bile duct cancer in adulthood, making surgical intervention crucial. Dr. Na, the lead surgeon, employed a cutting-edge single-port robotic surgery system to minimize scarring and enhance precision, performing the entire procedure through a single incision.

Operating on such young patients requires extreme delicacy, Dr. Na explained. It must carefully dissect the cyst while protecting vital structures like the pancreas. After cyst removal, it needs to precisely connect the tiny bile duct, only 3-4 mm in size, to the intestine. The smaller organs and confined surgical space in pediatric patients demand meticulous technique.

To overcome these challenges, Dr. Na utilized advanced three dimensional (3D) high-definition imaging and articulated instruments. This technology allowed for careful navigation of the narrow surgical area, enabling the team to remove the abnormally enlarged bile duct and gallbladder. They then reconstructed the connection between the liver’s bile duct and the intestine, ensuring normal bile flow.

Despite the patient’s significantly enlarged and twisted choledochal cyst, the surgery was a success. The child is now recovering well. Dr. Na affirmed that they’re committed to enhancing patient safety through precise, tailored treatments and minimally invasive surgeries. The goal is to reduce the burden on both patients and their families.

Korea University Guro Hospital is at the forefront of complex pediatric surgeries. The institution has established a state-of-the-art minimally invasive surgical infrastructure, with a focus on pediatric surgery including robotic procedures.
