Persistent bad breath despite regular brushing and mouthwash use may signal underlying gum disease rather than a simple hygiene issue, medical experts said April 7.

According to the medical community, gum disease, or periodontal disease, is not limited to swollen or bleeding gums but involves the gradual destruction of tissues that support teeth, including gums and alveolar bone, due to bacterial infection.

After eating, food debris, proteins in saliva and oral bacteria remain in the mouth. When combined, they form a sticky film on tooth surfaces known as plaque.

If plaque is not removed, it hardens into tartar, which has a rough surface that allows bacteria to accumulate more easily and promotes further plaque buildup.

As tartar and bacteria continue to accumulate around the gums, inflammation develops. Gums may become swollen, red and prone to bleeding during brushing. In some cases, the gums begin to detach from the teeth, forming deep pockets known as periodontal pockets.

These pockets create an environment where bacteria can thrive, leading to symptoms such as gum bleeding, swelling and bad breath.

As the condition progresses to moderate and advanced stages, gums may recede and teeth can become loose.

Scaling, a procedure that removes plaque and tartar from tooth surfaces, is the primary treatment for early-stage gum disease, with most cases of gingivitis improving through scaling alone.

For more advanced cases, root planing may be considered when periodontal pockets exceed 4 millimeters or when periodontitis has progressed.

Oh Min-seok, head of dentistry at Seran Hospital, said bad breath is one of the most common symptoms in patients with gum disease and often improves significantly or disappears with scaling, root planing and proper oral hygiene.

He added that gum bleeding during brushing is typically one of the first symptoms to improve, often within one to two weeks of treatment.

Oh also noted that tartar tends to accumulate in hard-to-reach areas such as the inner surfaces of lower front teeth, between molars and below the gum line, emphasizing the importance of regular scaling, flossing, use of interdental brushes and smoking cessation.

He recommended dental checkups for individuals experiencing related symptoms, noting that timely treatment can reduce bleeding, improve bad breath and prevent tooth loosening.
